Please remember that a parent is required to fill out the "PREPARTICIPATION PHYSICAL EVALUATION -- MEDICAL HISTORY" form prior to receiving a physical tomorrow. These will be provided tomorrow. A student cannot fill this out or sign it themselves. It is attached below for parents that cannot attend, but must be filled out before your child can receive a physical. The first form is for the parent to fill out, the second is for medical staff.

Physicals will be tomorrow (8/5) 8:30am-10:30am in the HS Gym.

All 7TH Graders participating in Athletics must get one.

8TH-HS students where applicable (as specified in previous posts/ Team App).
