The incident reporting form is not working via the link in Team App or the Belgravia Leisure website.

Staff without B-Connected access, can you please use the manual Incident Report Form located in your Facilities Team App ‘People’ section until further notice.

You can download and print this form via the Desktop version of your Facilities Team App should you need

Staff with B-Connected access can continue to use the form on B-Connected.
